A useful pilot has a specific question: whether a defined workflow can improve without unacceptable quality, risk or operational cost. Without that question, the pilot cannot inform an implementation decision.
The current state was never measured
If the organisation did not measure the prior process, it cannot distinguish an interesting prototype from an operational improvement.
No one owns adoption
Technology changes work. A process owner, review model and team enablement plan are as important as the system output.
